Instructions for Paperwork:
• Please complete the Intake Packet for your age group.  Please read and sign the forms as you were instructed. Please discuss any questions or concerns you have with your counselor.
• Please complete these forms prior to your first appointment.
• Please read the Welcome Packet, which is for your files. The Welcome Packet includes Maddox Family Counseling Privacy Notice in compliance with HIPAA.

Please contact the office at 205-922-4926 and request a client link to fill out electronic forms needed for your Maddox Family Counseling chart. If this is your first appointment, please fill out all forms before your appointment, as your counselor will not be able to meet with you until all forms are received and processed. 
 
Please complete and submit all forms at least 24 hours in advance of your appointment time.

Teletherapy FAQ

A. Teletherapy is an outpatient counseling session that is conducted via phone or video conferencing. It allows you and your therapist to visit remotely when circumstances prevent in-person attendance.

A. Yes! Research shows that teletherapy sessions can be just as effective as in-person sessions.

A. Yes! Teens, adults, and younger children (10 & up) can benefit from teletherapy services. The advancements of video conferencing has made teletherapy sessions almost identical to in-person sessions.

A. Yes! We ask that all clients find a quiet private place to conduct their session to ensure confidentiality on their end. No sessions or images are recorded by the therapists

A. New clients should call (205) 922-4926 to speak with an intake counselor.

Returning clients can visit the Client Portal or call (205) 922-4926 to schedule your next session.
